Derek Jones D.O. FAMILY MEDICINE
Derek Jones, D.O., attended Pikeville School of Osteopathic Medicine, Pikeville, Ky. He completed his internship and residency at Westview Hospital, Indianapolis, IN and is board certified by the American Osteopathic Board of Family Physicians.
Karem Menezes, M.D., earned her medical degree from Montemorelos University's School of Medicine in Montemorelos, Mexico. She completeed a pediatric residency and her pediatric internship at Miami Children's Hosptial in addition to a prior pediatric residency at Monterrey Institute of Technology and higher Education in Monterrey, Mexico. Dr. Menezes also completed an internship at Silvestre Adventist Hostpial in Rio de Janerio
Kari Shields M.D. FAMILY MEDICINE
Kari Shields, M.D., is certified by the American Board of Family Practice. She attended medical school at the University of Louisville, Louisville, Ky. and completed internship and residency at Marshall University, Huntington, W.Va.

Insurance and Payment Information
We accept most insurance plans including Medicare. Co-payments are required at the time of your visit. If you are a self paying patient, payment is expected in full at the time of service. We accept cash, checks, VISA, MasterCard and Discover.
Emergencies
If you have an emergency, the OLBH Emergency Department operates 24 hours a day. If your situation is not emergent, but is urgent and cannot wait for an appointment, OLBH also provides urgent care conveniently located at:

Prescriptions Refills
Patients are responsible for monitoring prescription refills. Please make the physician and nurse aware of the need for refills at the time of your visit. If you are not scheduled for an appointment but need a prescription refill, please call the office during business hours and leave a message for your nurse. We request 48 hours to process refill requests.
Please remember that in certain situations appropriate medical care requires the benefit of a patient examination before a prescription can be filled.
