Falls are the leading cause of injury deaths among older adults. More than one-third of people ages 65 and older fall each year, and the majority of these falls occur in or around the home. Bellefonte HomeCare Services offers practical solutions for patients who are at risk for falls though a fall prevention program, including:     

  • Patient assessment that measures balance and gait    

  • Home safety assessment to identify hazards that could result in a fall    

  • Medication review to determine risk for falls    

  • Instruction in safety precautions and proper use of equipment such as walkers and crutches

  • Exercise program

Bellefonte HomeCare Services staff works closely with patients and their physicians to help them attain goals that will reduce the risk of falls and improve their quality of life. Patients who complete the fall prevention program will learn the following:    

  • How to create a home environment that minimizes the risk for falls    

  • What effects medications have on risk for falls    

  • How to increase strength and balance through exercise    

  • What steps to take in the event of a fall

The falls prevention program is designed to identify patients at risk for falls and provide intervention when indicated. Each patient’s ability, mobility and environment are evaluated. Based on the findings and the patient’s activity tolerance, Bellefonte HomeCare Services staff will work with the patient’s physician and other health care professionals to determine the most effective level of treatment.
