Whole person health -- wholeness of body, mind and spirit -- is the
mission of Community HealthWatch Partnership, caring for people through
the integration of faith and healing.
CHWP partners with area congregations and other community organizations
to facilitate the development and support of health and wellness programs.
The program extends the hospital's mission of being "Good help to
those in need" by promoting disease management and prevention programs
and education; providing on-going training and support; promoting
healthy lifestyles and spirituality; and assisting with the development

Chauffeur Service Benefits Community
Curb to curb transportation service is just a phone
call away thanks to a
free-non emergency patient chauffeur serviced offered by Our Lady
of
Bellefonte Hospital's Community HealthWatch Partnership (CHWP).
In keeping with the hospital's mission of being "Good Help
to those in
need," CHWP operates a medical transport service equipped
with a wheelchair
lift. It provides service for patients who require minimal assistance
and
are utilizing OLBH services, such as outpatient testing, cancer
therapies,
same day surgeries, behavioral health services, cardiac and pulmonary
rehabilitation or visiting the OLBH campus physician offices.
The goal of the service is to provide transportation to those with
no other
means of transportation to and from appointments. It is being offered
free
of charge as a way to help those on a limited income who otherwise
may have
had to spend money on cab fares to appointments. Donations are
accepted and appreciated.
The hours of the patient chauffeur service are on a as needed basis.
Patients must reside within a 25-mile radius of the hospital. To
