of Bellefonte Hospital Outreach Centers provide residents with access to
primary care physicians, doctors of osteopathic medicine, pediatrics, physician
assistants, nurse practitioners and quality nurses. The centers' healthcare

Our Lady of Bellefonte Hospital Outreach Centers are located in Cannonsburg,
Flatwoods, Grayson and South Shore, Kentucky.
Augustinus Lobach, M.D., OLBH Outreach Center medical director and family physician,
describes the hospital's outreach centers as perfect settings for family practice
physicians. "It is not unusual for us to treat entire families at the centers.
